I believe it only casts Regen II when you are fighting DCs and III while fighting Ts and above.

Fight DC or better mobs. Auto will regen you with the highest level it knows.

Not correct, it only uses lower teir if recast is not ready, such as if the tank changes from one person in pt to other and just used regen 3 on first tank, it may use regen 2 on 2nd person.

Correct, although DC mobs dont always trigger it either.

You are right now that i think about it.

Mobs with large lvl ranges can have it triggering on one mob and not at all on a mob of the same type right next to it.

I shoulda said high DC and better. My bad there.

Well... a few weeks ago I was talking with another Puppetmaster and we both noticed something not quite right with the automaton's regen casting behavior after the previous update. Just a few days ago I did a little 50ish level synced party and had to use Soulsoother. The automaton's behavior seemed to be, "if the person who currently has the most enmity is injured but not below the cure threshold, I cast regen." However, after learning Regen III this behavior seemed non-existent. I don't know what triggers it to cast Regen III since it happens so rarely, but something about it did feel misplaced.

Which mage attachments did you have equipped during this odd behavior? And that is for the OP as well.

I've heard about a few ppl fighting mobs that should have forced Regens and it didn't happen, but no one can ever tell what their setup is.

And i can never replicate this when i try. The closest i have gotten is like was stated in an above post.

Mage-wise i always have mana booster, scanner, tranquilizer, dmg gauge, and optic fiber on when healing. And Klax has been doing same as he always does.

Just the usual. Damage Gauge, Mana Booster, ...nothing else that would really affect spell casting. All the same attachments when regen casting behavior was 'normal' as when it was not.

strange on my side everytime i fight DC+ mosltly t+, he always cast Regen III, and i always have 1 light 2 dark up.

and when someone else take hate, he never does regen II, he does III when timer is up

I think people don't see Regen often because by the time cast timer is ready they're at or below cure threshold again already, making the automaton choose to Cure instead of Regen. But yes, it has been confirmed that it only casts Regen on DC or harder enemies, and only on the person who has the most enmity when spell timer is ready and a Cure doesn't have priority.

You'll likely see Regen more often if you solo with NIN sub, or have a tank using shadows.

Agreed, usually in merit parties I'll pop out the soother if the RDM starts having difficulties. Then is when I see regen pretty often.
