Nice post. I would add a couple items.
In the food section I'd add Bison Steak for those with base attack over 375.
The Martial Knuckles are good for mnk/thf using SATA + DK since 100%TP is treated as 200% and DK damage is affected by TP.
For the mithra monk, you can add the RSE pants that give +1MND and 32MP. Mostly used for more MP especially if you doing alot of chi blasts and there is a mnk rotation so you don't get ballad often.
The water belt from Leviathan gives +3MND on watersday which is a nice option to carry in case it happens to be that day.

Thanks, Shul, I completely missed the Savage Loincloth there, not to mention Water Belt. I don't think I've ever fought a god on Watersday because our LS is paranoid about fighting on Lightsday (yet refuse to bring a Thief, go figure). It's good to see a valid reason to use Martial Knuckles, so I'll paste those in. As regards Bison Steak, though, I'm not really a fan. The only Monks who have that kind of base Attack rating are at end-game and, most likely, have a Bard, so I'm sure they can figure it out if they can get things like Bandomusha Kote and Shura Togi.

One piece of headgear you may want to include is the Super Ribbon.
It essentially offers +1 to all stats and aside from the Voyager's Sallet is likely giving you the best stat bonusses from 52 > 70'ish.

I just got Master Caster's Mitts from CP, I think you should add those for soloing/tanking options when /nin.
They're +2agi +10 evasion skill outside of nation's control. I can't believe I never saw them lol

Why Wagh Baghnahks?
They're a specialized weapon, one only used in certain situations, but in those situations, they shine. In my experience, the 14 Attack and latent Damage are not as good as Indra Katars, but they're close. Destroyers blow these things out of the water, but Waghs can be bought and Destroyers have to be fought for. Anyhow, the 14 Attack does help a bit, particularly in these days of nerfed food (I still remember when Meat Mithkabobs gave absurd Attack and don't understand why they changed that). The only time to use Waghs, though, is when a.) you don't have anything better and b.) you're in an Asuran Spam party in KRT. You're doing yourself and the party a disservice to use Wagh Baghnahks for anything other than spamming weapon skills, though.
Of course, for that very reason, I don't much like Waghs. I'm the weird person who would wait a round or two for one of the other Monks to fire off Asuran Fists, then I'd close Fusion or Fragmentation off them. Choice between a 900 damage Asuran Fists or a 700 damage Raging Fists + 500-700 damage Fusion chain and, if you have a smart Red Mage, a Fire III magic burst? Yeah... people shouldn't abandon the holy skillchain, it's entirely worth waiting three seconds for.
Anyhow, that's why Waghs: for when you can't get anything better. I did try to make as comprehensive (without going completely insane) list as possible.

Very Nice Onorgul. Though may I ask why you didn't put the T.M Hooks+2. It looks like your list are end-game list and with the last martial arts traits, T.M+2 shines.

I would like to add a new food for high end monks.
Hedgehog Pie....it's disgusting on how good it is. It's 4 am so i'll edit and post a link later...but "Ddddaaaaaammmmmmmmnnnnnnnn" is the only word that can describe it for melee.

hmmm, Alla's info doesn't synch with Somepage or mysterytour. It's lacking the +5 acc and the +55 hp (two of the more important factors to take it a notch above yellow curry/cor subs).

Is the Tiphia Sting a good ammo equipment? The Attack and Accuracy seems nice but I'm not sure on the -4 evasion. We have HP to spare and it seems good for monks subbing THF or WAR

It is absolutely the best ammo slot item. If you are worried you can macro it out or take it off when you pull hate, but I just leave it on.
